Our practice accepts patients 17 years and older. Referring physicians’ offices can make appointments for patients by calling our office at 404-303-0003 between 8:30AM and 5 PM Monday through Friday. It is very helpful if, in advance of the patient’s appointment, you will fax to our office at 404-303-0036 our Referral Form along with office notes, labs, and studies pertinent to the referral.
